Health and Safety

All children born after January 1, 1992 entering day care must have proof of hepatitis B vaccination. Parents are required to furnish an up-to-date Pre-School/Child Care Immunization Certificate (Form 3231) as proof of immunizations. Children who have completed hepatitis B series may be granted a 180 day waiver. Statement for Children lacking Hepatitis B Vaccine Series (Form 3115, Rev.1-97) certified by a licensed physician or health center will be accepted by the Center for those children who have completed all required immunizations except hepatitis B series. Form 3231 (rev. 1-97) is required when the child is fully immunized with all required vaccinations, including Hepatitis B Series.

So that we may best anticipate the needs of your child, we expect you to provide information concerning the child and any special health conditions and/or considerations such as: asthma, allergies, diabetes, epilepsy, heart disease, hyperactivity, etc... All students will be observed daily for any signs of illness. If any symptoms of illness appear during the day, the parent will be notified and expected to pick up the child immediately. Please notify the Academy at once if the child does have a communicable disease. Approval for re-admittance to school by a signed Physician will not be accepted if a student has signs or symptoms of a contagious disease. No child will be accepted nor allowed to remain at the center if the child has the equivalent of a 99 degree or higher oral temperature and other contagious symptoms. All families must keep an updated health record file.

MEDICAL EMERGENCY PROCEDURE
In case of accidental injury, we will make an immediate attempt to contact a parent. If we cannot reach you, we will call the child’s physician. If necessary, we will also call an ambulance, or paramedics. The Director or designated staff will be in charge and make all decisions about the care of the child. You will be expected to assume responsibility for any resultant expense not covered by insurance. The Academy will maintain a parent’s signed consent form for this provision.

PROTECTION OF CHILDREN IN THE EVENT OF EMERGENCIES
The Voices of Faith Academy practices fire drills on a regular basis. Fire drills are set apart by having a continuous bell. Students are instructed to line up and listen to the directions of the teacher. Students must file out to the assigned area in a neat and orderly fashion. Students must stay together with their teacher and wait for further instructions for the drill. Tornado drills are set apart by several intermittent short bells. Students are instructed to move into the hallway away from any glass or entryway of the building. Students will sit on the floor, with their backs up to the wall with their head covered. Fire and tornado drills will be posted in all classrooms. All staff is required to be familiar with these procedures. Drills are conducted periodically and a log is maintained. For safety reasons, we request that students and parents DO NOT RUN INSIDE THE BUILDING.
