Registration packets are available for families who wish to apply to the Daycare. Information regarding the Daycare can be given to parents over the telephone on an informal basis. Packets must be filled out in its entirety in order for the admissions staff to process them.

Applicants to the Daycare must provide the following information:
1. Application
2. Parent Payment Contract
3. Parental Agreement Contract
4. Emergency Medical Authorization
5. Immunization Record (Form 3231)

For the health, safety, and welfare of your child and that of the Daycare, we may require
you to provide additional information and complete forms concpe~iing your child. An individual file will be kept for each child enrolled in the Daycare. The contents of this file will be kept confidential and will be available for review by the parent or guardian upon request.

We expect the parents to keep all information on file updated including change of address, telephone numbers, name and place of employment. These changes will be dated and initialed by the parent and the Director .
